Todd J. Sontag, DO
Board Certified in Family Practice & Osteopathic Manipulation
Dr. Sontag graduated from Florida State University. He obtained his Medical Degree from Nova Southeastern University College of Osteopathic Medicine, where he also obtained a Masters Degree in Public Health. He completed his residency at St. Vincent’s Medical Center Jacksonville in 2008.
Dr. Sontag has practiced Family Medicine at Family Practice Associates since August of 2008. He is a member of the Duval County Medical Society and is fluent in medical Spanish.
Michael A. Edwards, DO
Board Certified in Family Practice & Osteopathic Manipulative Treatment
Dr. Edwards graduated from Villanova University. He obtained his Medical Degree from Nova Southeastern University of Osteopathic Medicine. Dr. Edwards completed his residency at St. Vincent’s Medical Center, Jacksonville. He joined Family Practice Associates in July, 2008.
Dr. Edwards is a native of Jacksonville where he lives with his wife Amy and their two children. He is a member of The American Osteopathic Association and The American College of Osteopathic Family Physicians.
W. Herman Sessions, MD
Board Certified in Family Practice
Dr. Sessions graduated from the Medical College of Georgia. He completed his residency training at the Naval Hospital in Charleston, S.C. and served on the teaching staff at the Family Practice Clinic, Naval Regional Medical Center, Jacksonville. Dr. Sessions entered private Family Practice in Orange Park in 1981, co-founded and has practiced medicine at Family Practice Associates since 1983. Dr. Sessions also has added qualifications in geriatric medicine.
Dr. Sessions and his wife, Pat, have resided in Orange Park since 1977 and have four grown children.
Richard J. Bultman, MD
Board Certified in Family Practice
Dr. Bultman graduated from the University of Michigan Medical School in 1969. He continued his training at Oakland Naval Hospital and the Aerospace Medical Institute in Pensacola. Dr. Bultman served as a Naval Flight Surgeon until 1974 when he entered private Family Practice in Orange Park. Besides being board certified in Family Medicine, Dr. Bultman is also certified in Sports Medicine and is a Senior Aviation Examiner for the FAA.
Delivering quality medical care to the whole family and the whole person, including early treatment and the prevention of disease, is his goal. Dr. Bultman believes this is done through attention to the physical, emotional and spiritual dimensions of his patients.
He and his wife Louise have three children and have enjoyed serving many families in Clay County. He is an avid tennis player and bicyclist. He enjoys playing the piano and traveling with Louise.
Luna I. Khadour, MD
Board Certified in Family Practice
Dr. Khadour is a graduate of the University of Bagdad, Iraq. She completed her residency at the University of Florida Jacksonville in June 2001. She joined Family Practice Associates in Orange Park August of 2001. Dr. Khadour is a member of the American Academy of Family Physicians. She is fluent in Arabic.
Dr. Khadour is married, has two children and resides in Jacksonville.
L. Michael Waters Jr., DO
Dr. Waters graduated from Florida Atlantic University. He obtained his Medical Degree from Nova Southeastern University College of Osteopathic Medicine where he also received the Samual J. Salman, D.O. Award in Family Medicine. Dr. Waters completed his residency at St. Vincent’s Medical Center Jacksonville in 2010.
Dr. Waters is a native of Florida. He resides in Jacksonville with his family. He is a member of The American Osteopathic Association and The American College of Osteopathic Family Physicians.
AJ Torres,RN, PA-C
Certified Physician's Assistant, Registered Nurse
A.J. has worked as a certified Physician Assistant for 14 years. He completed his B.S. in Medicine at the University of Florida in 1995. Prior to that, A.J. worked as a Registered Nurse at Memorial Medical Center Jacksonville for five years.
He joined Family Practice Associates two years ago. A.J. is married and has two daughters. He enjoys swimming, reading and running with his dogs.
Janet Underkofler, PA-C
Certified Physician's Assistant
Janet has been practicing as a Board Certified Physician Assistant for 24 years, including ten years of surgical experience. Janet graduated from the University of Florida in 1985. She worked for eight years in the Department of General Surgery at Mayo Clinic. Janet joined Family Practice Associates 14 years ago.
Originally from New York, Janet resides in Jacksonville with her husband Steve and their three children.
